To register for Scuola Italiana di Portland classes you will need to provide us with one the following Registration forms:

• Children’s Weekend/After School – The International School
• Children’s After School – Catlin Gabel School
• Preschool – ZCC
• Adult Beginning Italian – FUMC
• Adult Intermediate Italian – FUMC
• Adult Advanced Conversation Italian – FUMC
• Adult Beginning & Intermediate Italian – The International School

We also would appreciate a photographic release form.

Finally, for children, we require an emergency contact form.

• Student Emergency Record Form
